Proof of Residency (Light, Gas, Water Bill, Tax Statement or Lease Agreement)

Immunization Records                  

Child’s Social Security Card          

Birth Certificate

Parent’s or Legal Guardian’s Picture ID               

Proof of Income  

 

 

Children must be 4 years old by Sept 1^st to enroll in Pre-K 4.
