If you were a patient of Dr. Steve Oates, Jr. this is to notify you that his office will be closing Jan. 30, 2015.

 

If you would like to pick up your medical records, you may do so at the office until the 30th.

 

The hours are Monday-Thursday, 9 to 12 p.m. and 2 to 4 p.m. and Friday, 9 to 12 p.m. The records will be available free of charge.

 

Please call (936) 598-5626 before you come so we can have them ready.

 

It is with heavy heart that this has to be done. We will notify you regarding future details as we get them.
